Download the appropriate link above, fill out + sign pages 1-7, then return the completed forms along with your COI to [email protected]

All customers without established credit terms (COD Status) will be required to complete the Credit Card Authorization form on page 3 of the the New Account Forms + Policies files located in the links above.

Required Insurance (COI) terms
  1. A-Camera Cinema Rentals listed as “Loss Payee” and “Additional Insured."
  2. Liability and Rented Equipment coverage.
  3. Liability coverage alone does not cover rented equipment - it MUST state the Production Package, Rented Equipment policy or Inland Marine policy with amounts and deductible listed. Replacement Cost Basis. Unattended Vehicle Exclusion not accepted.
  4. Coverage is worldwide including transit. List amount and deductible.

1. When Are You Open?

Hours: Monday-Friday 9AM – 5PM
After hours /weekend gear pick up or drop off- $100
Weekend Prep fees – $350

2. What Are Your Insurance Requirements?

  • A-Camera Cinema Rental named as Loss Payee and Additional Insured -Liability and Rented Equipment Policy required-Coverage and Deductible amount shown.
  • No unattended vehicle exclusion.
  • Global coverage required for outside of US rental.

3. When Is Gear Pick Up & Drop Off?

Gear Pick Up : After 2:00 PM
Gear Drop off: By 10AM following wrap
